ADA UPdates Hypertension Management Recommendations for Diabetics
The American Diabetes Association (ADA) has updated recommendations for hypertension and blood pressure monitoring in diabetic patients. The blood pressure of certain patients, the ADA says, should be measured at every routine clinical-care visit. Further, multiple readings should be made on those patients with an elevated pressure.
The ADA has not significantly updated its guidance since 2003. "There is 'unequivocal evidence' that supports targeting BP goal to
Standing BP measurements should be taken routinely and in the case of elevated readings, confirmation readings should also be taken.
Initial standing blood pressure measures should be taken during the initial evaluation of the patient and then for every follow-on. These recommendations replace earlier recommendations that had BP readings only taking place at the original evaluation and in more limited circumstances when follow-up evaluations came.
In patients with a high risk of cardiovascular disease, a lower blood-pressure target of
The ADA's Professional Practice Committee reviewed 137 clinical trials and meta-analyses to draw new conclusions as to recommended practices. They noted that in diabetic clinics, blood pressure monitoring is often taken with less care than it could be. Given the close links between hypertension, high blood pressure, and diabetes, it's important, they emphasize, that blood pressure absolutely be a part of the diabetic treatment regimen. They recommend American Heart Association guidelines for blood pressure measurements and treatments.
